Well, you asked for a review. And asked. And apparently really, really wanted one, so it would be rude of me not to oblige.
I ordered Mounjaro and was subsequently informed that The Family Chemist had decided they would no longer prescribe it to anyone who had undergone bariatric surgery — in my case, gastric bypass eight years ago.
I accepted their decision. Their pharmacy, their prescribing policy, their choice. Refund received. End of story. Or so I naively thought.
What I found rather interesting was the explanation that this decision was based on “new evidence and safety concerns”. Having actually looked into this afterwards, I cannot find a blanket UK contraindication stating that Mounjaro cannot be prescribed simply because somebody has previously had bariatric surgery. Previous gastric bypass is not listed as a contraindication in the current UK prescribing information, and GLP-1/GIP treatments are actually being studied and used for weight regain following bariatric surgery.
Of course, post-bariatric patients may require appropriate nutritional and clinical monitoring. That is rather different from implying that eight-year-old bariatric surgery suddenly makes Mounjaro inherently unsuitable. If this is simply The Family Chemist’s own remote-prescribing policy, perhaps saying exactly that would be rather clearer than dressing it up as though a new national contraindication has appeared overnight.
Extra points for sending me an explanation about the risks in “post-gastric sleeve patients” when I had a gastric bypass. Different operation, but never mind — presumably Ctrl+C and Ctrl+V were having a particularly productive day.
Anyway, I accepted the decision, took my refund and unsubscribed from their emails.
The Family Chemist, however, appears to be having considerably more difficulty accepting our separation than I am.
Newsletter arrives. Unsubscribe.
Another email. Unsubscribe again.
“Recommend us to a friend and receive £40 off!” — absolutely marvellous. £40 off what exactly? The medication you’ve just decided you won’t prescribe to me? 😂 Unsubscribe. Delete.
And now, today’s masterpiece: “Please review us.”
You know what? Fine. 😂
I genuinely wasn’t going to leave a review. The order was cancelled, the money was refunded and I had moved on with my life. But after repeatedly ignoring my attempts to unsubscribe, you have now personally delivered an invitation to publicly describe the experience.
So here we are.
Perhaps alongside regularly reviewing the “latest clinical guidance”, somebody could occasionally review the marketing unsubscribe list too.
Thank you for your persistence. You finally got the review you worked so incredibly hard for.

We would like to clarify an important point regarding the clinical decision made in your case.
At no stage did we state that previous bariatric surgery is a blanket UK contraindication to Mounjaro. A medicine not being formally contraindicated does not automatically mean that it is appropriate for every patient or that every prescriber is obliged to prescribe it.
As a regulated online pharmacy, our prescribing clinicians are responsible for determining which patients can be treated safely within the limitations of a remote prescribing service. Following a review of the available clinical evidence, associated risks and our prescribing protocols, a decision was made that patients with certain previous bariatric procedures fall outside the criteria under which our clinicians are currently prepared to prescribe this treatment remotely.
This was a clinical safety decision, not a suggestion that a new national contraindication had appeared. We fully appreciate that other healthcare providers may assess and manage such patients differently, particularly where additional face-to-face assessment, nutritional monitoring or specialist follow-up is available.
We do, however, acknowledge your point regarding the reference to gastric sleeve surgery when you had informed us that you had undergone gastric bypass surgery. The distinction should have been reflected accurately in our communication, and we apologise for that error.
Once our clinicians determined that we could not safely prescribe within our current criteria, your order was cancelled and a full refund was issued. While we appreciate that this outcome was disappointing, we will not compromise our clinical prescribing criteria simply to fulfil an order.
We appreciate that you may disagree with our prescribing criteria, but patient safety and the professional judgement of our prescribing clinicians must take priority. We would always rather decline and refund an order where our clinicians are not satisfied that treatment can be appropriately managed through our service than prescribe outside the parameters they consider safe.
